To register for a particular class, fill out this form and then print it and mail to one of the addresses below.

There is a limit of 15 participants in each class. Class positions will be reserved on a first-come first-serve basis. For session 1, registrations must be received by 01 May 2001, and for session 2, by 01 Aug. 2001.
